The natal chart is the map of the sky at the moment of your birth, calculated on the coordinates of the place of birth and the exact time. It shows where Sun, Moon, planets and key points (ascendant, descendant, midheaven, imum coeli) were in the twelve astrological houses. It is the fundamental tool of Western astrology and the basis for more sophisticated readings like transits, solar return and synastry.
Three pieces of information are needed: date of birth (day, month, year), time of birth as precise as possible (the registries of many countries record it on the birth certificate) and place of birth (city and country). Time is particularly important: ten minutes of error can shift the ascendant by one degree, and half an hour can change the sign.
Positions of the ten traditionally considered planets (Sun, Moon, Mercury, Venus, Mars, Jupiter, Saturn, Uranus, Neptune, Pluto) in the twelve houses and twelve signs. The aspects, that is the angular relations between planets (conjunction 0°, sextile 60°, square 90°, trine 120°, opposition 180°). Special points: ascendant (cusp of the 1st house), midheaven (cusp of the 10th), lunar nodes, part of fortune.

Without time, prefer to use the tool for sun sign or moon sign (which only needs date and place). For the full natal chart, with no time, some astrologers set the symbolic time 12:00 and mark the ascendant as "unreliable".
The natal chart is the snapshot of the sky at your birth: unique and stable. The horoscope (daily, weekly, monthly) describes the astrological "weather" for a given sign or for your profile, and is dynamic.
Astrology describes symbolic cycles (transits) that many interpret as inner seasons. It does not guarantee specific events: how a transit manifests always depends on the person and the context.
